A useful support plan is specific enough for a busy school day. Identify the concern, listen to the student's perspective, agree on the next step, and name the adult responsible for following up. Record when that check-in should happen and what would indicate that the support needs to change.
A teacher may notice participation changing. A counselor may help clarify a concern. A principal may need to remove a scheduling or staffing barrier. Each person needs the context appropriate to their role, and the student needs to know whom they can approach.
Kaiden's intended workflow keeps those responsibilities and follow-up conversations connected. A completed check-in does not automatically mean the student's need has been resolved. Staff review what the student says, what they observe, and whether the support remains appropriate.
